Book an advance care planning presentation in Melbourne

We believe advance care planning is a heartfelt conversation and a personal statement that goes way beyond filling in a form. We want to enable every Australian to make the best choices for their life and health care, based on their values and preferences. The Advance Care Planning Volunteer Community Ambassadors lead an Introduction to Advance Care Planning information session.

Advance Care Planning Volunteer Community Ambassadors have been a part of the Advance Care Planning Australia team since 2012. In this time, they have provided 350 presentations to over 7000 people. Our Community Ambassadors lead an introductory, free 40-minute presentation. The presentation aims to increase community understanding of advance care planning, what the benefits are, and how to undertake the process. These presentations are delivered to a variety of clubs, groups, events and organisations.

At this stage, we are only able to facilitate these presentations in Melbourne.

Advance care planning can assist people to express their hopes for different stages in their illness, and make these preferences clear to people, like those who act as their substitute decision-maker, to ensure that their preferences are met.

What we will do

We will:

  • Assist your organisation to ensure our information session will meet the needs of your attendees
  • Work with you to advertise or promote your information session.
  • Provide equipment for the session to run smoothly (if required).
