“Advance care planning offers an important opportunity for people with a life-limiting illness to take control of their own health care by communicating their preferences and ensuring that the care they receive aligns with their personal values. I am excited to be a part of this successful health reform program and to promote the significant role it plays in achieving positive outcomes.”
