Even though many of us would love to live well into our 90s, the reality is a third of us will die before the age of 75 and a lot of us are likely to spend some of our final years unable to communicate.

With this in mind, Australians are being urged to spend some time thinking about how we would like our final period of life to play out.

Life Matters' host, Hilary Harper is joined by Dr Chris Moy, a General Practitioner and ambassador for National Advance Care Planning Week and Julie Cheney who shares her personal experience about making tough medical decisions for her husband.
