We are looking for participants to interview for a collection of case studies that show examples of how advance care planning has made a positive difference to a person's care outcomes.
You could be a person who has their own Advance Care Plan or Directive, or a health care professional, aged care worker or carer who has experienced the benefits of advance care planning. We'd love to hear from you!
Share your story with us
Advance Care Planning Australia and Decision Assist would love to hear from people from diverse backgrounds, professions and roles who have an experience with advance care planning.
We want to know more about how advance care planning has helped people to achieve health and personal care in line with their goals, values and preferences. Sharing authentic experiences from people in the community helps us to continue to improve advance care planning practice.
Who can participate?
- Individuals who have an experience of advance care planning - either directly or as a family member
- Health care professionals
- Aged care workers
What will it involve?
If you agree to participate, our interviewer will call you for one or more phone interviews. We will ask you to sign a consent form allowing us to publish your personal story on our website and elsewhere as required.
How will the interview be used?
Advance Care Planning Australia and Decision Assist will share these personal stories in our education programs and on our website as case studies to illustrate the benefits of advance care planning.
Phone interviews will take place throughout July and August 2017.
