Australia’s population is ageing. Well-coordinated and appropriate health care is a key priority. It's becoming increasingly important for the health care sector to better understand advance care planning, which supports person-centred care, aligned with an individual’s values, goals and preferences.
Presented by Advance Care Planning Australia, QUT and Queensland Health, this Brisbane-based seminar will be run by recognised leaders in the field of advance care planning in Australia.
The topics covered include advance care planning across sectors, legal considerations and implementation. The event is recommended for professionals leading and implementing advance care planning in health services, residential aged care and primary care. To find out more about the program see the seminar agenda.
