Presented by Advance Care Planning Australia, the Advance Project and HammondCare, this Sydney-based seminar will be run by recognised leaders in the field of advance care planning in Australia.
The topics covered include advance care planning across sectors, legal considerations and local initiatives. The event is recommended for professionals leading and implementing advance care planning in health services, residential aged care and primary care.
To get the most out of this event prior ACP knowledge is essential.
This event will not cover the basics of advance care planning. It is most suitable for people that have mastered the basics and are leading others or championing ACP in their organisation. The presentations will rely on evidence-based models and relay research data and implementation strategies.
