When a person can no longer make their own medical treatment decisions, their support person or carer may be required to act as their substitute decision-maker.

A recent study, found that the Australian public have a low to moderate understanding of what it means to be a substitute decision-maker. Participants reported wanting more information and support.

At Advance Care Planning Australia, we have developed a free support program for substitute decision-makers. We provide assistance to families, friends, and carers, who may have to make the medical treatment decisions for someone they love. Anyone can contact us directly for support and health professionals can refer substitute decision-makers to this program through a written referral.

This program will provide substitute decision-makers assistance with:

  • understanding and accepting the role
  • having the conversation and exploring values and preferences
  • completing documentation, if required
  • understanding when the role takes effect and making medical treatment decisions
