Across three inspiring days, we explored advance care planning through cultural, personal, organisational, system, and societal lenses. Now the task is to take what we’ve learned – the research, the tools, the stories, and the partnerships – and bring them into our daily work. Nurture the friendships and networks you’ve built at ACP-i 2025, and carry forward the momentum. Together, we will be working on the call to actions so we can transform advance care planning from aspiration into lived reality for all people, everywhere.
