Advance Care Planning Improvement Toolkit
The Australian Government Department of Health and Ageing funded the development of the Advance Care Planning Improvement (ACPI) Toolkit. It was delivered by Advance Care Planning Australia in 2022.
The ACPI Toolkit was developed to support the uptake and quality of advance care planning and advance care directives across Australia. It helps ensure that more Australians have choice and control over their future treatment decisions.
ACPI Toolkit overview
The health and aged care workforce and service providers have responsibilities in legislation and common law to implement advance care planning and enact advance care directives.
The ACPI Toolkit provides health and aged care services with resources to support organisational advance care planning uptake, performance monitoring, and quality improvement. The toolkit complies with the National Safety and Quality Health Service Standards and Aged Care Quality Standards.
The ACPI Toolkit has been developed to support health organisations and aged care services:
- provide consistent ACP assessment tools
- collect a standardised and robust ACP dataset
- document and measure the quality of ACP in their healthcare settings.
Using the toolkit
The ACPI Toolkit includes a guide outlining how to use it within your organisation, 3 separate audit tools, and reporting templates that align with each audit tool.
The 3 audit tools assess an organisation’s:
- Advance care planning systems
- Advance care planning documents in health records
- Advance care planning consumer experience.
Annual auditing is recommended for best practice.
Tailored for your state or territory
Laws governing medical treatment decisions and advance care planning differ across states and territories. Because of this, a customised version of the toolkit was developed for each jurisdiction. They can be downloaded below.
